Use of oral anticoagulants is ubiquitous with increasing use of direct oral anticoagulants (DOACs) in lieu of warfarin. As well, there are many options for reversal of these agents. Yet, there are no PMGs related to which reversal agent to use. A PMG is needed to guide therapy for warfarin, dabigatran, and oral Xa inhibitors.
